CarePort Health Delivers Interoperability Solution to Help Hospitals Comply with New CMS Regulations

As an intermediary, CarePort reduces compliance burden and provides hospitals, physicians and post-acute providers with unmatched access to well-established, national network

BOSTON--()--CarePort Health, a market leader in care transitions software, today announced a comprehensive interoperability solution to help hospitals achieve compliance with new Conditions of Participation (CoPs) mandating electronic notifications as part of the Centers for Medicare & Medicaid Services’ (CMS) Interoperability and Patient Access final rule. While deadlines for compliance have been extended due to COVID-19, the pandemic highlights the importance of interoperability and real-time patient event notifications to better track and monitor patients as they move through the care continuum.

The new CoPs support post-discharge transitions by requiring that hospitals, or an intermediary on the hospital’s behalf – like CarePort – send electronic patient event (ADT) notifications to post-acute providers, primary care physicians and other organizations when a patient is admitted, discharged or transferred. Though CMS has extended the implementation timeline for the ADT notification CoPs, hospitals must still have policies and procedures in place by spring 2021.

CarePort is uniquely positioned to support these new CMS requirements: 40% of hospital discharges to post-acute providers in the U.S. already flow through the CarePort platform every year. CarePort serves as an intermediary connecting an established national network of thousands of hospitals with physicians and post-acute providers across the country. Fueled by robust data sources and integrated with all leading EMRs, the CarePort platform has visibility into admission, discharge and transfer events across a network of 1,000 hospitals in 43 states, and processes 10 million patient events daily.

Enabling electronic notifications to ensure compliance with CMS’s forthcoming CoP mandate is foundational to how CarePort’s suite of solutions works. To support the interoperability requirements, CarePort delivers notifications to a national network of primary care physicians and post-acute providers on behalf of hospitals. The customizable patient event notifications incorporate clinical information to help manage the referral process and provide critical patient context. This actionable information ensures providers know how to best care for patients within specific settings.

“It is time for cross-continuum care coordination to be made a priority, and CarePort’s extensive network is uniquely positioned to foster increased communication and transparency between hospitals, physicians and post-acute providers,” said Lissy Hu, M.D., CarePort CEO and founder. “Despite extended timelines resulting from COVID-19, this pandemic has made evident – more than ever before – the critical importance of interoperability and contextual, actionable alerts to better inform patient care. We’ve developed a solution to make it as easy as possible for providers to comply with these regulations and focus on their mission and competencies – healthcare.”
