US Post Acute Care LLC to provide clinically driven support services

TACOMA, Wash.--()--US Post Acute Care LLC announced today the start of operations as a Management Services Organization, providing value-added service to clinical practice groups specializing in the care of patients in the post-acute setting. The new company was founded by Darren Swenson MD, CEO of Swenson Healthcare PLLC, a leading post-acute practice group with operations in the Pacific Northwest.

An organization with a national footprint, US Post Acute Care provides a full array of management services including billing, recruiting, training, compliance, analytics, and practice development. Traditionally, post-acute clinicians relied on available support from multispecialty groups or health systems not designed to meet the specific needs of post-acute medicine. US Post Acute Care structures its services offering both to serve the needs as well as leverage the capabilities of its clients: the physicians, nurse practitioners and physician assistants dedicated to improving the quality of patients in the post-acute setting.

US Post Acute Care has developed a proprietary clinical excellence program called SmartPAC™. Developed over the past two years by the clinical leadership team at Swenson Healthcare, the SmartPAC program is a solution specific to the needs of the post-acute medical practitioner. Currently in use with more than 3,000 patients in over 30 post-acute settings, SmartPAC™ is designed to deliver a comprehensive system of post-acute care, enabling practitioners to deliver high-quality care clinically proven to improve patient outcomes.
