(NYSE: HUM) have entered into a value-based care network agreement designed to offer a coordinated, patient-centered experience for Humana Medicare Advantage Health Maintenance Organization (HMO) members at four new Dedicated centers in Philadelphia.
Dedicated offers affordable, “high-touch” primary care at centers that are focused on Medicare beneficiaries and are physician-led. The agreement gives Humana Medicare Advantage HMO members in-network access at Dedicated’s first four locations in Philadelphia: Olney (5675 N. Front Street); Mayfair (6431 Sackett Street); North Philadelphia (2101 West Lehigh Avenue); and West Philadelphia (1575 N. 52nd Street).
High-touch primary care for Medicare-eligible seniors reduces hospital admissions, decreases healthcare costs, and improves patient use of preventive medication. Dedicated centers focus on detecting and managing high-risk diseases while providing preventive care and services as well, including:
- Daily walk-in and same-day appointments when needed
- On-site specialist doctors, medication dispensing, x-rays, tests, and screenings
- Substantively more face-to-face time with physicians
- Well-being classes, such as tai chi and yoga
