As a result, the Brentwood, Tennessee-based RegionalCare Hospital Partners – which includes nine rural hospitals and affiliated physician practices located across several states – expects to experience the improved outcomes and reduced costs vital to success under value-based programs.

To achieve this goal, the health system relied on RelayHealth’s Connected Care solution to create a record that consists of data culled from multiple vendors’ electronic health record (EHR) systems installed across the healthcare enterprise. In addition to creating a single view, RegionalCare is enabling patients to access this information through the RelayHealth patient portal. The portal allows patients to use a single sign-on to view their comprehensive records – instead of snippets drawn from individual systems – and lets them directly connect with providers. Such engagement can help physicians and patients work together toward improved health outcomes.
