NAPA, Calif.--(BUSINESS WIRE)--SCAN Health Plan announced today that it is expanding its Medicare Advantage plan into Napa and Sonoma counties, effective January 1, 2015. SCAN is one of the nation’s largest not-for-profit Medicare Advantage plans and currently serves members in Marin, San Francisco, San Joaquin and Santa Clara counties as well as eight additional counties in Southern California and Arizona.
Unlike most Medicare Advantage plans, SCAN has no commercial members, which allows it to dedicate 100 percent of its energy and programs toward serving older adults across the wide spectrum of aging. Its focus is to provide affordable and accessible quality healthcare to seniors and to continue to look for innovative ways to serve the healthcare needs of Medicare beneficiaries.

Partnering with SCAN as its contracted provider in Napa and Sonoma is Meritage Medical Network, a healthcare network of physicians, ancillary services, physical therapy, wellness classes and more. Contracted hospitals include Petaluma Valley Hospital, Queen of the Valley Medical Center, Santa Rosa Memorial Hospital, and Sonoma Valley Hospital.
SCAN is offering three health plans in Napa and Sonoma counties. “SCAN Classic (HMO)” is a Medicare Advantage Prescription Drug (MAPD) plan with no monthly premiums. The plan includes $5 co-pays for primary care physician office visits, and $15 co-pays for visits to specialists along with no cost for annual exams, routine eye exams, lab services, X-rays and immunizations. Seniors and others on Medicare can enroll in SCAN Classic during the annual enrollment period of Oct. 15 – Dec. 7.
SCAN is also offering two Special Needs Plans (SNP) – for individuals with chronic conditions. SCAN Heart First (HMO SNP) is for people with cardiovascular disease or congestive heart failure, and the SCAN Balance (HMO SNP) plan is for individuals with diabetes. SCAN Heart First and SCAN Balance are available with $20 monthly premiums, $5 co-pays for primary care physician office visits, and $15 co-pays for visits to specialists. Insulin is available through these plans with no co-pay. Medicare allows beneficiaries to enroll in SNPs year-round, not just during the annual enrollment period.
In addition to these benefits, SCAN brings to local seniors a commitment to excellent customer service. As part of that commitment, all SCAN employees participate in SCAN’s Trading Ages “aging-sensitivity program” to help them understand the process of aging and better identify with members’ needs.
SCAN Health Plan is an HMO plan with a Medicare contract offered in California by SCAN Health Plan and in Arizona by SCAN Health Plan Arizona. Enrollment in SCAN Health Plan depends on contract renewal. SCAN Balance (HMO SNP) is available to anyone with Medicare who has been diagnosed with diabetes. Heart First (HMO SNP) is available to anyone with Medicare who has been diagnosed with certain cardiovascular conditions.
