NASHVILLE, Tenn. & MARSHFIELD, Wis.--(BUSINESS WIRE)--naviHealth and Security Health Plan of Wisconsin, Inc., today announced their collaboration in managing post-acute care services for Security Health Plan’s Medicare Advantage and commercial members. The program will use naviHealth’s decision-support technology and field-based clinicians to deliver high-touch care management services to Security Health Plan members receiving skilled nursing facility, long-term acute care and inpatient rehabilitation services.
naviHealth’s care managers will engage and assess Security Health members in the hospital to educate the members and their caregivers on their options for care and rehabilitation. After discharge, naviHealth will continue to work with patients and their care teams in post-acute facilities to update care plans and monitor progress against their personalized goals.
