North American Medical Management Signs Contract with Humana to Expand to Arizona
Will Extend Coordinated Care Model to Humana Medicare HMO Members
PHOENIX--(BUSINESS WIRE)--North American Medical Management Arizona, Inc. (“NAMM”) and Humana Inc. (NYSE: HUM) today announced a long-term agreement under which NAMM will organize a network of contracted private-practice physicians to care for Humana Gold Plus HMO members in the greater Phoenix area.

Humana Gold Plus HMO plans provide Medicare-eligible individuals in Maricopa County with affordable alternatives to original Medicare or a Medicare supplement plan. Members can enjoy broad coverage, including all the benefits of original Medicare and prescription drug coverage. The annual enrollment period to sign up for the Humana Gold Plus HMO plan is November 15 through December 31 for an effective date of benefit beginning January 1, 2010. Individuals already participating in the Medicare Part D program may enroll through March 31, 2010.
