Enhanced interoperability helps hospitals and health systems achieve value-based care and better health outcomes

BOSTON--()--CarePort, powered by WellSky, a market leader in care transitions, today announced that through partnerships, the CarePort platform has significantly expanded its data connections across the U.S., providing expanded visibility into a growing number of providers and increased care coordination capabilities across the continuum. With these enhancements, CarePort is powering smarter care at a lower cost to achieve elevated health outcomes for patients and their families.

Now connected to 17 heath information exchanges (HIEs) and data sources in the country and growing, CarePort simplifies the sharing of important patient data. CarePort’s clients can access admission, discharge, and transfer (ADT) event notifications and Continuity of Care Documents (CCDs) within the CarePort platform, allowing for:

  • More complete patient utilization histories
  • Additional clinical data including labs, medications, results, and procedure information
  • Expanded alerting capabilities from additional sites of care
  • Improved accuracy for reporting through enhanced analytics
  • Improved transitional care management and value-based workflows
