Together, PrescribeWellness and CSS will enable community pharmacists to expand services, reach more patients, improve Star Ratings, and bring greater value through combined and coordinated face-to-face and phone-based medication therapy management (MTM) services, in the first end-to-end solution for healthcare payers.

Putting patients first
Both PrescribeWellness and CSS maintain a dedicated focus on patient care, delivering the business around patient need and better health outcomes.
With this new partnership, PrescribeWellness' network of 10,000 community pharmacies will perform medication reconciliation in person with patients, and CSS will complete the session with the patient over the phone, giving health payers "ground and air" support as they fight the epidemic of chronic disease in the U.S. by means of comprehensive patient engagement.

Enabling true collaboration
PrescribeWellness is practice management system-agnostic, interfacing with over 50 pharmacy management systems to analyze dispensing data in real time, allowing for true collaboration across communities, giving the pharmacy team insight into the needs of their patients, and streamlining the medication reconciliation process. Participating pharmacies do not have to be using the same practice management system to participate and recognize the same benefits.
Using PrescribeWellness' PrescribeCare tool, community pharmacies can perform medication reconciliation, document encounters, share with other providers, and submit for billing. MTMPath, CSS's medication and care management platform, is specifically configured as the electronic health record (EHR) for pharmacists. MTMPath was designed to manage a medication-related clinical program in any care environment and facilitate systematic, high- volume medication and care management.
