Vivify Health Collaborates with American Heart Association to Extend Education and Support for Heart and Stroke Patients after Hospital Discharge

PLANO, Texas--()--Vivify Health now offers the American Heart Association’s (AHA) Connected Heart Health Initiative, enabling direct patient usage of the AHA’s science-based care plans. This powerful combination of evidence-based Care Plans and technology is the result of a five-year relationship between Vivify Health and American Heart Association.

Vivify Health provides a Remote Care Management Platform using multiple modalities of interaction for Remote Patient Monitoring, Chronic Care Management, Transitions of Care Services and Population Health. Connected Heart Health is the American Heart Association’s new initiative to deliver patient-centered care beyond the clinic. AHA Care Plans focus on post-acute/preventative care in several key areas including heart failure, coronary artery disease, cardiac rehabilitation, atrial fibrillation, stroke rehabilitation, hypertension and diabetes.
