NASHVILLE, Tenn. & PLANO, Texas.--(BUSINESS WIRE)--naviHealth and Golden Living today announced a partnership to enhance the post-acute care service delivery for patients admitted to Golden Living’s skilled nursing facilities around the country. The clinical and strategic collaboration will utilize naviHealth’s decision support technology, post-acute care analytics platform and network of clinicians to enhance patient outcomes and improve the transition of care for patients who desire to return home. The partnership is intended to support the value-focused paradigm shift taking place in reducing the cost of health care while improving the total patient care experience.
“Through this collaboration with Golden Living, naviHealth continues to positively impact post-acute care on a large scale by implementing innovative solutions that enhance outcomes while driving down costs,” said Clay Richards, naviHealth president and chief executive officer. “Our partnership with Golden Living to deliver comprehensive care management services to its eligible skilled nursing facility providers is hopefully the first of many such alliances we can forge to improve the delivery and management of post-acute care across the country.”
Under the partnership, naviHealth and Golden Living will enhance coordination of patient care and outcomes through a number of program elements including: the sharing of data, clinical collaboration on ER diversion and readmission programs, and the appropriate inclusion of Golden Living providers in any proprietary provider networks established by naviHealth.
“As a leader in the post-acute care industry, we look forward to working with a like-minded innovator that shares our desire to enhance the total health care experience for our patients and residents on a national level,” said Neil Kurtz, Golden Living president and chief executive officer. “We believe that our collaboration with naviHealth will bring significant value to the patients within our provider network. The partnership between naviHealth and Golden Living is one example of how providers and payors are working together in response to our country’s health care reform.”
Post-acute care accounts for 73 percent of regional variation in Medicare spending largely due to significant practice variation and a general lack of clear, evidence-based plans for patients transitioning from the hospital.1 Skilled nursing care is fundamental to driving improved post-acute outcomes for patients, and also accounts for the largest amount of post-acute utilization and spend. naviHealth seeks to partner with skilled providers, like Golden Living, to align stakeholders through its field-based and virtual service model to provide data-driven and coherent care plans that drive better outcomes at lower costs.
