naviHealth and Golden Living Announce National Partnership to Enhance Post-Acute Care Experience and Patient Outcomes

NASHVILLE, Tenn. & PLANO, Texas.--()--naviHealth and Golden Living today announced a partnership to enhance the post-acute care service delivery for patients admitted to Golden Living’s skilled nursing facilities around the country. The clinical and strategic collaboration will utilize naviHealth’s decision support technology, post-acute care analytics platform and network of clinicians to enhance patient outcomes and improve the transition of care for patients who desire to return home. The partnership is intended to support the value-focused paradigm shift taking place in reducing the cost of health care while improving the total patient care experience.

Under the partnership, naviHealth and Golden Living will enhance coordination of patient care and outcomes through a number of program elements including: the sharing of data, clinical collaboration on ER diversion and readmission programs, and the appropriate inclusion of Golden Living providers in any proprietary provider networks established by naviHealth.
