One of only five companies selected by AVIA and UHC

SAN DIEGO--()--Skylight® Healthcare Systems, developer of the industry’s most comprehensive patient engagement platform, is one of five companies selected by AVIA and UHC to participate in the 2nd Annual UHC Startup Challenge taking place Friday, Oct. 24 at UHC’s 2014 Annual Conference in Las Vegas.

Skylight Healthcare will be presenting the company’s advanced and comprehensive interactive patient engagement platform, Skylight Interactive. The platform engages both patients and providers throughout the continuum of care to reduce the fractures that exist in the health care system and effectively improve patient engagement, provider satisfaction, the quality of care, and the health of communities.

Using any communication modality, Skylight’s platform notifies both patients and providers of the information necessary to foster patient engagement and improve communication between patients and their health care providers. It also communicates essential patient health information across providers to ensure the entire care team is informed of patient results and activities.

Patients can set their communication preferences to ensure that they receive essential health care information and instructions via their preferred communication device and messaging method.

Providers are then able to receive automated notifications on patient health information. For example, test results of a recently hospitalized patient can be sent to the patient’s primary care physician and the hospital’s case manager. This ensures that patients are treated appropriately, costly mistakes are avoided and readmissions are reduced.

Skylight’s platform improves engagement and coordination in health care by acting as a hub to gather health information and then disseminate that information to patients and clinicians at the appropriate time, location, and communication mode.
