Loopback Analytics to Provide Technology Platform for Florida Atlantic University’s INTERACT™ Program

Solution manages patient populations and care transitions in real-time

DALLAS--()--Loopback Analytics, a care transitions management platform provider, has established a license to integrate INTERACT™, a quality improvement program developed by the nationally renowned experts at Florida Atlantic University. INTERACT™ (Interventions to Reduce Acute Care Transfers) focuses on the management of acute change in patient condition at skilled nursing facilities across the nation.

“INTERACT within the Loopback Analytics platform empowers providers to ensure the delivery of consistent quality care,” said Neil Smiley, president and CEO of Loopback Analytics. “Dr. Ouslander’s pioneering work combined with our advance technology has helped to evolve INTERACT from a concept into a viable and comprehensive quality improvement program. This collaboration and innovation, demonstrates how the addition of the Loopback Analytics platform to an existing, proven concept can take it to another level of success.”

The Loopback Analytics cloud-based platform helps skilled nursing facilities using the INTERACT™, program manage patient populations and care transitions by improving tracking and root cause analyses for hospitalization rates, as well as improve communication with local hospital providers. In addition, new tools have been included to enable proper medication reconciliation for medication orders when patients transfer from a hospital to a facility.

“The inclusion of the Loopback Analytics platform provides clinicians with a more organized process to document patient assessments and offers a powerful benefit in terms of the ability to monitor critical care processes and outcomes in real time,” said Joseph Ouslander, M.D., INTERACT™ project director, professor and senior associate dean for geriatric programs at the Charles E. Schmidt College of Medicine at Florida Atlantic University. “The ease of access to real-time information keeps the treating physician informed of changes in a patient’s condition to support appropriate modifications to the care regimen in order to avoid hospitalization. As a result, patient satisfaction is increased and costs are contained.”
