OVERLAND PARK, Kan.--(BUSINESS WIRE)--ViaQuest, Inc., a leading provider of behavioral health, home health, hospice, and developmental disabilities services, has chosen Netsmart CareManager™ as the care management and care coordination solution for its planned Health Home.
The company’s Clinical Services Division will utilize CareManager as part of the process to attain Health Home certification from the State of Ohio and to share approved health information with Health Home provider participants and the state’s proposed health information exchange.
Health Homes are comprised of teams of healthcare professionals responsible for coordinating care across multiple providers. By integrating primary care and behavioral healthcare, and coordinating care regardless of where patients receive it, Health Homes are designed to help improve quality of care and health outcomes while also bending the cost curve.
“Netsmart’s deep understanding of coordinated care and the solutions they’ve already developed made it an easy choice for us as we prepare to launch our Health Home,” said Chris Wolf, chief operating officer, ViaQuest. “CareManager will allow us to significantly streamline our workflow, track outcomes more effectively and capture data to help us measure the effectiveness of our services.”
Netsmart CareManager provides an open, platform-agnostic approach because it is designed to connect to any electronic health record (EHR)/electronic medical record (EMR). This comprehensive view into a patient’s health record provides broader insight and visibility into the activity taking place within the Health Home, allowing providers to ensure their patients receive the right care at the most comfortable location at the appropriate time. CareManager also facilitates care coordination by allowing providers within a Health Home to follow a shared care plan, exchange authorized health data, and track clinical quality measures and outcomes across the network.
“We are engaged in more federally-designated Health Home initiatives than any other care coordination solutions provider, and are pleased to partner with ViaQuest as they offer transformative, coordinated care with a Health Home,” said Michael Valentine, chief executive officer, Netsmart. “Our CareManager solution is emerging as the defining model for providing behavioral health-centered Health Homes with immediate, consistent access to current data and enabling clinicians to provide the most appropriate care with optimal outcomes.”
Netsmart is the knowledge and technology partner for Health Homes in Iowa, New York, Ohio, Tennessee, Washington and Wisconsin.
In addition to CareManager, ViaQuest also utilizes Netsmart’s myAvatar™ CareRecord™ (electronic health record), OrderConnect™ e-prescribing service, and document management services.
