Health Dialog’s Latest HEDIS and Stars Support Program Drives Improved Clinical Outcomes and Addresses Critical Quality Measures for Health Plans

Customized approach delivers impactful solutions that raise ratings

BOSTON--()--Health Dialog today announced the latest version of their HEDIS and Stars support program to help health plans successfully navigate today’s healthcare challenges and improve their quality scores. The program offerings include a framework for the development of implementation strategies that improve clinical outcomes.

Health Dialog’s HEDIS and Stars support programs address critical quality measure areas, including prevention and screening, respiratory conditions, cardiovascular conditions, diabetes, and musculoskeletal conditions. The HEDIS program also supports health plan efforts to meet CAHPS® Health Plan Survey scores. Both programs consist of three key program components – identification, outreach, and direct coach education support. Health Dialog’s registered nurse Health Coaches specialize in behavior change and provide the decision support and knowledge that enables health plans to address member barriers and improve care while lowering costs.

In 2012, a health plan that used Health Dialog’s HEDIS program to close diabetes care gaps saw a 7.3% increase in HbA1C tests, 6.9% increase in Lipid tests and 4.9% increase in Microalbumin tests. In addition, a regional Medicare Advantage plan using Health Dialog’s Stars program achieved demonstrably higher results - 25% of women targeted for osteoporosis management closed care gaps, and 15% of habitually non-adherent women obtained a breast cancer screening.
