Priority Health and naviHealth partner to improve rehabilitation outcomes for patients after a hospital visit

GRAND RAPIDS, Mich.--()--Priority Health and naviHealth today announced the launch of a new program to further improve clinical and therapy services provided to members after a hospital visit. The new agreement will utilize Priority Health’s nurses and naviHealth’s decision support technology and field-based care managers to work with members and their caregivers to proactively set goals for rehabilitation after a hospital visit; creating highly personalized and effective care.

“Most patients leave the hospital without a clear plan or guide for what’s next, leading to large variations in the care they receive, costs and outcomes,” said Jay LaBine, M.D., chief medical officer for Priority Health. “Priority Health is focused on improving the health and lives of the people we serve. This new program with naviHealth will support the incredible work our team already delivers by creating resources for our members and their families to ensure they have a clear understanding of what to expect after a hospital visit and have access to personalized care to achieve the best possible health outcomes.”

Members of Priority Health already experience significantly lower rates of unnecessary hospitalizations compared to those enrolled in health plans in Michigan and nationally. A study by Johns Hopkins University and the Alliance of Community Health Plans revealed that members of Priority Health’s Medicare Advantage plans are three times less likely to be readmitted to the hospital than those enrolled in traditional Medicare due to Priority Health’s established patient support programs.

Rehabilitation services account for a significant portion of health care costs in the United States. According to a congressional advisory panel, last year $62 billion, one out of every six dollars Medicare spent in the traditional fee-for-service program, went to nursing and therapy for patients in rehabilitation facilities, nursing homes, and long-term care hospitals. Medicare per capita spending on these services has grown by 5 percent a year or faster in 34 of the nation’s 50 largest hospital markets in recent years, according to Kaiser Health News.

The program begins in the hospital setting with Priority Health and naviHealth working closely with the discharge staff to connect with Priority Health members and their families before they leave. Priority Health nurses and naviHealth Care Managers then work with members and their care teams to continually update their care plan and monitor progress toward achieving their goals.
