Munroe Regional Medical Center, a 421-bed hospital that manages over 35,000 inpatient and over 100,000 emergency room visits annually among a predominantly elderly population with substantial medication utilization, recently implemented a strategy where home med lists were documented for 98.8% of patients, med rec was completed on more than 96.3% of patients the average time for reconciliation was reduced to five minutes, 34 seconds with 34% taking less than two minutes.
