BOSTON--(BUSINESS WIRE)--Health Dialog Services Corporation today announced it will provide care management services to Integrated Healthcare, the fully integrated, point & click-based consumer-driven health platform for small to mid-sized companies. The Health Dialog program, which includes Health Coaching and related support, will be offered as part of Integrated Healthcare’s point-and-click-based consumer-driven healthcare platform product. The new services will be integrated with other information tools and financial incentives such as health savings accounts, health risk assessments, smoking cessation programs, and weight management.

Health Dialog Health Coaches, who are registered nurses, dieticians, and respiratory therapists, work with individuals and family members by telephone and online to help manage chronic conditions, significant medical decisions, and general health and wellness.
