Humana and Boulder Community Health Partner to Launch Accountable Care Arrangement

BOULDER, Colo.--()--Humana Inc. (NYSE: HUM) and Boulder Community Health announced today a new Accountable Care arrangement and population-health partnership. The agreement supports continued efforts to provide improved health and coordinated care for individuals and families purchasing insurance in Boulder County through Connect for Health Colorado and through Humana commercial health maintenance organization (HMO) health plans for employers.

The new partnership is expected to result in more personalized care for Humana membership with an emphasis on individual preventive care, chronic conditions and disease management. The agreement offers improved standards of care to those choosing Humana’s HMO plans through, but not limited to, Colorado’s insurance marketplace, which continues enrollment through Feb. 15, 2015.

Under terms of the accountable care agreement, Humana members will have in-network access to the inpatient, outpatient and emergency care services at Foothills Hospital and Boulder Community Hospital as well as the urgent care center, physician offices and special medical services at Mapleton Center and at Community Medical Center. More than 128 primary care physicians and specialists affiliated with Boulder Community Health are also now in-network to Humana members.

“BCH is dedicated to providing local access to high-quality, comprehensive medical care for Humana members,” said David P. Gehant, President/CEO of Boulder Community Health. “We’ve established a reputation for offering the latest medical innovations and evolving our services to meet our community’s changing health care needs.”

The value-based arrangement emphasizes quality outcomes with a focus on care coordination and leverages measures as defined by the National Committee for Quality Assurance (NCQA) Healthcare Effectiveness Data and Information Set (HEDIS) for diabetes care and treatment, breast cancer screenings, colorectal cancer screenings and high-risk medication.

“These types of partnerships are a critical step for Humana in Colorado as we look to create a system that works in a more coordinated way,” says Jeremy Gaskill, Regional Vice President, of Market Development for Humana’s West Central Region. “With a tremendous scope of services and highly integrated model, Boulder Community Health is an ideal partner as we continue developing improved quality care and health care experiences for our members.”
